Extra Care STI Clinic

Poverello's EXTRA Care Clinic is a program designed to provide integrated healthcare services to individuals with complex medical and social needs who are living with chronic conditions. The intervention provided by the program involves a comprehensive, team-based approach to care that addresses both the medical and social determinants of health. The free clinic's multidisciplinary team consists of Nurse Practitioners, Physicians, Social Workers, nutritionists, peer counselors, and case managers. Patients receive primary medical care, mental health & nutritional support, assistance with accessing housing, education, employment, and other social services. The team works to address social determinants of health in a direct way.
